You should have received an email with your user name and password. If not, please call our office.
It is important that you access the patient portal to complete your forms and medical history. Patient portal downloads directly into your medical records for the most up to date information.
Note: Patient forms must be completed on the PATIENT PORTAL WEBSITE or PRINT these forms and bring Completed Forms to your appointment.

1. Login using password you received by email.
2. Go to the Left Column
A. My Heath – complete the sections listed under this tab.
B. Check My Forms to make sure they have all been completed.
You will receive an online check in request via text the night before your appointment.
IF YOU ARE UNABLE TO COMPLETE THESE FORMS PRIOR TO YOUR APPOINTMENT, PLEASE ARRIVE 30 MINUTES EARLY FOR YOUR APPOINTMENT TO USE OUR PATIENT PORTAL KIOSK COMPUTERS.
Please bring insurance cards, copay and/or deductible along with your referral if applicable to your appointment.
If you are unable to keep your appointment for any reason, kindly let us know at least 24 hours in advance.
